IMMEDIATE BREAST RECONSTRUCTION WITH SUBPECTORAL SILICONE IMPLANT AFTER SUBCUTANEOUS MASTECTOMY

Abstract

|
Subcutaneous mastectomy is becoming an operation of choice in premalignant and other breast pathology Although it has not been constructed as a cosmetic procedure, an acceptable final appearance has been a major demand by women who want to get rid of diseased breast tissue and yet maintain a satisfactory body image.
Breast reconstruction after subcutaneous mastectomy can be achieved with silicone implant, musculocutaneous flap or dermofat graft.
But we have found the resulting appearance to be unsatisfactory too often because of high incidence of capsular contracture and indentation and stepladder deformity on the upper pole and nipple area of breast.
This is a case report of immediate breast reconstruction after bilateral subcutaneous mastectomy in paraffinoma(2 cases) and fibrocystic disease with ptotic breast (I case). We had good results by the following methods
(1) Subpectural augmentation with silicone implant beneath the pectoralis and serratus anterior muscle immediately after subcutaneous mastectomy.
(2) Fur the selection of implant volume to augment breast, we used the tissue expander under the muscles during operation.
(3) In the large ptotic breast, we had simultaneous reduction mammoplasty with free nippie-areolar graft.
